为自身或他人申报的第三方申报人、代表客户申报的申报代办人，以及申报大量交易且已申请或被要求以电子方式申报的个人，均须以电子方式提交申报。

 Electronic Claim submission is required for third party filers filing on their own behalf or on behalf of others as well as to claim preparers filing on behalf of clients, or to individuals filing a large number of transactions who have requested or have been requested to file claims electronically.

所有电子申报均须填写在本说明后附的模板中。

 ALL ELECTRONIC CLAIMS SUBMISSIONS MUST BE ENTERED ON THE TEMPLATE THAT FOLLOWS THESE INSTRUCTIONS.

电子申报必须采用本文件指定的格式提交。

Electronic claim submissions MUST be submitted in the required format, specified in this document.

每个独立法律主体应提交1份申报。

 One Claim is to be submitted for each separate legal entity.

每个独立法律主体均应单独提交申报，且仅可提交1份（例如，代表共同所有人提出的申报，不应包含其中某一共同所有人单独进行的交易；代表个人提出的申报，不应将其个人退休账户交易与仅以其个人名义进行的交易合并申报）。

A separate Claim, but only one Claim, should be submitted for each separate legal entity (e.g., a Claim on behalf of joint owners should not include separate transactions of just one of the joint owners, and a Claim on behalf of an individual should not combine his or her IRA transactions with transactions made solely in the individual’s name).

每份申报均须提供该申报所涉证券实益所有人正确、完整、未经缩写的姓名或名称。

 You MUST provide the correct complete, unabbreviated name of the beneficial owner(s) of the security supporting each Claim.

未提供该信息的，您提交的申报可能被拒绝。

Your submission may be rejected if you do not provide this information.

请勿使用尊称或头衔（如先生、夫人、女士、博士、上尉、中士），也不要在实益所有人姓名或名称前加注“FBO”（为他人利益持有）或类似字样。

Do not include honorifics (e.g., Mr., Mrs., Ms., Dr., Capt., Sgt.) and do not put “FBO” or the like in front of the beneficial owner’s name.

请注意：受托人以受托人身份行事时，并非证券的实益所有人；应填写信托的完整名称，其后列明各受托人的完整姓名或名称。

PLEASE NOTE: A trustee, in that capacity, is not the beneficial owner of the security; the full name of the trust should be entered followed by the full name(s) of the trustee(s).

信托名称与受托人姓名或名称之间应以逗号分隔。

A comma should separate the name of the trust and the name(s) of the trustee(s).

 The required documentation set forth below MUST be submitted with your electronic file.

您必须填写1份申领证明表（以下简称“申报表”）中的申领人身份信息页（第一部分第4页）、收款方式选择表（第7页）及证明与签名页（第三部分第8—9页）；该表将作为电子文件中所有申报的“汇总”或“总”申报表。

 You MUST complete the Claimant Identification Page, (Part I, page 4), Payment Election Form (Page 7) and Certification and Signature Page, (Part III pages 8-9) of a single Proof of Claim Form (“Claim Form”), which will serve as an "umbrella" or “master” claim form for all Claims in your electronic file.

申报表必须由签字权限核验文件中列明的经授权签字人签署，并注明其身份（职务名称）。

 The Claim Form MUST be signed by an authorized signatory who is listed on your signature verification document and state the capacity (job title) of the signatory.

如代表多名实益所有人申报，请在实益所有人姓名或名称栏填写“Various Beneficial Owners”（多名实益所有人）。

 If you are filing on behalf of multiple beneficial owners, use the term "Various Beneficial Owners" for the beneficial owners’ names.

但如上所述，电子申报中的每份申报仍必须提供各实益所有人正确、完整、未经缩写且不含尊称或头衔的姓名或名称。

However, as noted above, the correct complete unabbreviated name of each beneficial owner (without honorifics) MUST be provided for each Claim in your electronic Claim submission.

如您是为自身或其他实益所有人申报的机构，或代表实益所有人申报的申报代办人，必须提供文件，证明签署申报表及任何补充文件的个人已获授权代表您签字。

 If you are an institution filing on your own behalf or on behalf of other beneficial owners or a claim preparer filing on behalf of beneficial owners, you MUST provide a document verifying that the individual who signs the Claim Form and any supplemental documents is authorized to sign on your behalf.

Some common types of documents that fulfill this requirement include the following (this list is not exclusive and non-US entities may have different documents that fulfill this requirement.

 If you are an institution filing on your own behalf or on behalf of other beneficial owners or a claim preparer filing on behalf of beneficial owners, you MUST provide a notarized affidavit or signed letter on your firm’s letterhead which meets the following criteria:

— 确认文件中独立申报的数量及交易笔数。

- Confirms the number of distinct Claims and transactions in your file.

— 说明文件中每份申报的数据来源。

- Sets forth the source of the data for each Claim included in your file.

— 证明文件中每份申报的数据真实、准确。

- Attests to the truth and accuracy of the data for each Claim in your file.

— 由贵单位签字权限核验文件中列明的经授权签字人签署，并注明签字人的身份及联系方式。

- Is executed by an authorized signatory who is listed on your firm’s signature verification document and specifies both the capacity and contact information of that signatory.

如您是代表自身以外的实益所有人申报的机构或申报代办人，必须提供现行有效的文件，证明您已获授权代表证券实益所有人提交并签署申报。

 If you are an institution or claim preparer filing on behalf of beneficial owners other than yourself, you must provide a current document verifying that you are authorized to file and sign claims on behalf of the beneficial owners of the securities.

- Notarized affidavit or signed letter on your company's letterhead confirming your authority to file and sign claims on behalf of each client for whom you filed a claim.

该文件必须由公司高级职员签署，该高级职员亦须列于您的签字权限核验文件中，文件还须注明签字人的身份及联系方式。

It must be executed by an officer of the company who is also listed on your signature verification document and reference the capacity and contact information of the signatory.

JND Legal Administration及／或美国证券交易委员会可视情况要求申报人提供补充文件，以支持所提交的申报。

 Filers may be requested, as deemed appropriate by JND Legal Administration and/or the United States Securities and Exchange Commission, to provide additional documentation to support the Claims submitted.

该项数据完整性审计旨在核验数据文件的整体完整性。

This data integrity audit is designed to verify the overall integrity of a data file.

因此，您必须提供所有要求提交的文件，且所提供的文件必须具有独立性。

Accordingly, you MUST provide all the requested documentation and the documentation provided MUST be independent in nature.

即使您已提供信函或宣誓书，证明随电子文件最初提交的数据真实、准确，我们仍可能要求补充具体书面证据，例如您以支票或电汇付款的证明、认购协议或其他类似文件。

 Even if you provided a letter/affidavit attesting to the truth and accuracy of the data you initially submitted with your electronic file, we may ask for additional specific documentary evidence, which may proof of your payment via check, wire, subscription agreement, or other similar documents.

如您的文件被选中接受数据完整性审计，您必须提供所有要求提交的信息。

If your file is selected for a data integrity audit, you MUST provide all the requested information.

只有在JND Legal Administration向您发送电子邮件确认收到文件后，电子文件才视为已提交。

Electronic files will not be deemed submitted unless JND Legal Administration sends you an email acknowledging receipt of your file.

收到该邮件前，请勿自行认定文件已被收到。

Do not assume that your file has been received until you receive this email.

如提交后10日内未收到此类邮件，应通过[email protected]联系电子申报部门，查询文件情况并确认其已收到。

If you do not receive such an email within 10 days of your submission, you should contact the electronic filing department at [email protected] to inquire about your file and confirm it was received.

Before entering any information on the E-Claim filing template, carefully examine the data required as set forth in the "Data Layout" section located in the Electronic Filing Template.
